We would like to thank you for choosing Southwest Ohio ENT Specialists as your provider. We have written this policy to keep you informed of our current office policies.
Payment for services at the office is due at the time the actual services are rendered. We accept cash, checks, MasterCard, Visa, Discover and American Express.
To facilitate your reimbursement, we will be happy to help process your insurance claims by filing with your primary or secondary insurance companies. We accept assignment for all Medicare patients. However, you will be responsible for the deductible and 20% of the Medicare approved amount.
The filing of claims is a courtesy that we extend to all our patients; however all charges remain your responsibility from the date the services are rendered.

Our practice is committed to maintaining the privacy of your personally identifiable health information.
In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you.
We also are required by law to provide you with this notice of our legal duties and privacy practices concerning your identifiable health information.
By law, we must follow the terms of the notice of privacy practices that we have in effect at the time.
If you have been referred to our practice by your primary care doctor, he or she will be advised of our treatment recommendations as they arise.
Because we are a specialty practice, many insurance companies require a written referral from your primary care doctor. It is your responsibility to make sure our office receives the referral. If you are here for an appointment and your referral has not been received, you will be asked to reschedule your appointment.
There is no greater compliment than being asked to care for one of your friends or family members. Our appointment book is always open for someone you know.
